About the role:

We have a part time position available for a Social Connections Coordinator in the community aged care services division at our Eaglemont office. The days are negotiable to fill three days per week from 8:30AM – 5:00PM. This is a temporary contract role until the 30th June 2016.

The role is responsible for the planning and implementation of social connection opportunities (including coordination of leisure and activity programs) based on our client’s individual interests and needs and connecting them to an appropriate community resource.

About you:

To be successful in this role you will have previous relevant experience as a lifestyle assistant within aged care and hold a Certificate IV in Leisure and Lifestyle or equivalent.

You will show enthusiasm and commitment to work as part of a team that focuses on providing services and support that meet the needs of each individual person.

Along with a creative flair, you will have the knowledge and ability to access a range of available community activities, as well as experience in developing and facilitating small activity groups.
